About us
We are a multidisciplinary health and social care team bringing care to people's homes in the Oeste region and around Leiria. We work with families, not just for them.

Oeste Med Assist grew out of something health and social care professionals kept seeing: access to healthcare, especially primary care, remains difficult for many families.
Appointments were missed for lack of transport, treatments were delayed because nobody could provide them at home, and families were managing situations nobody had prepared them for. That gap is where the project began.
We started by bringing together people already doing good work at home: doctors, nurses, physiotherapists, occupational therapists, social workers and social educators. Then we organised what was missing, with a written plan, one named contact and clear communication with the family.
Today we connect healthcare with home support providers and social institutions, because health and daily life cannot be solved separately. Our network keeps growing, driven by the wish to serve those who need care and those who give it.
We do not promise everything to everyone. We say what we do, who we work with and how fast we answer.
Assessment by qualified professionals, a written care plan and supervision by a coordinating nurse.
A team that knows the municipalities where it works and speaks with the family in the language they feel safe in.
The same named contact throughout the care, with an agreed replacement when needed.
A written quote before care begins, with no joining fee and no minimum term.
Every care plan has named people responsible for it. It is not a call centre: it is who decides with the family and answers for the plan.
